Overview

Semaglutide and tirzepatide are FDA-approved GLP-1 (and, for tirzepatide, GLP-1 / GIP) receptor agonists with the strongest clinical evidence base of any compound on this menu. Landmark trials include STEP 1 (semaglutide: mean weight change −14.9% vs −2.4% placebo at 68 weeks), SURMOUNT-1 (tirzepatide 15 mg: up to 22.5% weight reduction at 72 weeks), and the SELECT cardiovascular outcomes trial (semaglutide reduced major adverse cardiovascular events by 20%). Compounded versions exist in a separate, more constrained regulatory category since 2025.

Regulatory & sourcing

Semaglutide (Wegovy/Ozempic) and tirzepatide (Zepbound/Mounjaro) are FDA-approved. Tirzepatide shortage resolved December 19, 2024; semaglutide shortage resolved February 21, 2025. On April 30, 2026, the FDA proposed to permanently exclude semaglutide, tirzepatide, and liraglutide from the 503B bulks list — closing the large-scale outsourcing-facility compounding pathway. Patient-specific 503A compounding remains possible only with documented individual clinical need.
